John W. De Ford, was a resident of Fayette County, Pennsylvania, when he enlisted on June 20, 1861, as 2nd lieutenant, and was commissioned into the 11th Pennsylvania Reserve Volunteers, also known as the 40th Pennsylvania Infantry.
